• A copy of your LABORATORY or IMAGING reports pertaining to the reason for visit- If not forwarded in advance
• LIST OF MEDICATIONS
• Glucose log for patients with Diabetes
• Most updated Health Insurance Card
• Proper Referral (if required)
• A list of your questions or concerns
• Form of payment – We accept Credit Card, Debit Card, American Express, check and Cash.
